Steven C. Greenwald, Msw, Lcsw

Steven is a graduate of the University of Illinois School of Social Work (Urbana-Champaign). He holds both a Masters's and Bachelor’s degree in, Social Work. Mr. Greenwald's experience includes extensive work with geriatric, psychiatric, and medically impaired populations in residential, in-patient, and out-patient settings. He provides direction and consultation to many long-term care facilities in several states. Mr. Greenwald is a popular instructor at seminars and workshops addressing the delivery of social work and behavioral health care services in skilled care, assisted living, special dementia care environments. His expertise includes: Educating staff on best practices to comply with the Centers for Medicare/Medicaid (CMS) standards, JCAHO accreditation, care plan development, staff development/management/evaluation, quality improvement/performance improvement, deficiency refutations, and plans of correction. Mr. Greenwald is a nationally known author and the editor of the SocialWork Consultation Group Letter, a quarterly newsletter distributed to nursing facilities in the United States and Canada. He has developed over 200 PowerPoint presentations to facilitate the education of long-term care staff. He has received professional acclaim and recognition for his long-term care resource books. Mr. Greenwald received certification from the Academy of Certified Social Workers (ACSW) early in his career and he is a Licensed Clinical Social Worker (LCSW). He is an advisory board member of the Activity Therapists Association of Illinois.

 

Catherine Bradley, Msw, Lcsw, ACSW

Cathy is our Director of Long-Term Care Consultation Central Illinois Region and has been a member of our team and family for over twenty-eight years. Cathy is a graduate of the University of Illinois School of Social Work (Urbana-Champaign). Initially she completed her BSW at UIUC and following six years providing medical social work at Proctor Hospital, she completed her MSW. Upon graduation, Cathy worked at the Galena Park Home for nine years. For a few years, Cathy worked in her family business and started consulting to LTC facilities independently. Cathy is a Licensed Clinical Social Worker (LCSW) and a member of The Academy of Certified Social Workers (ACSW). She is an active member of NASW and was honored as the Social Worker of the Year for the Peoria District. She has co-authored scholarly articles for Social Work Today and for Topics in Geriatric Rehabilitation. Cathy is an excellent presenter on many topics including Mental Health, Therapeutic Recreation, Behavior Management, and Skills Training. In her spare time, Cathy is a marathon runner and has been an active participant in the Memphis to Peoria St. Jude Run for over 25 years.

 

Zoya Zackavec, Msw, Lcsw

Zoya Zackavec is the Regional Director of Social Work Consultation for SwCG, Inc. and has been an integral part of our team for over 11 years. She is a Licensed Clinical Social Worker (LCSW) with experience and expertise in medical social work, gerontology and mental health. Zoya received her BA in Communications at the University of Pennsylvania. She went on to get her MSW at Loyola University Chicago, saying it was by far her proudest moment. Zoya is proficient in long-tern care regulatory standards and best practices, advocacy for the at-risk elderly, persons with severe mental illness, advance medical directives, nursing home consultation, supervision and Point Click Care (PCC). Zoya is fluent in English, Russian and Spanish. Zoya is also an outstanding fencer from a competitive and renowned fencing family.

 

Erica Hill, Msw. Lcsw

Erica Hill has been a part of the SocialWork Consultation Group, Inc. team for many years, originally joining our group in 2006. She graduated from the University of Nebraska in 2001 with her MSW. As a long-term care consultant, she brings expertise in clinical social work program supervision, quality assurance review, diagnostic assessment skills, psychosocial programming and care plan development. Her experience is with SNF's, IMD/ICF's, Mental/Behavioral Health, Substance Use Disorders, Dementia Care, and Supportive/Independent Living Centers. Erica is a dynamic speaker and facilitates workshops/in-services, guest lectures, and Internship Supervision. 

 

Kylie Roxas-Bogoff, MACP, LPC

Kylie Roxas is credentialed as a Licensed Professional Counselor (LPC) in the state of Illinois. She earned her Master of Arts degree in Counseling Psychology at the Chicago School of Professional Psychology. Kylie has a special interest in health psychology, particularly with health promotion and wellness among the elder community. Kylie has previously worked in a psychiatric hospital for clients with severe mental illness, senior community centers and long-term care settings. This afforded her the opportunity to understand the aging process and its psychological impact to one's mental health. Kylie has been part of SWCG, Inc. family for over 5 years and is a valued member of our team. She provides interim staffing support, supervision training for new hires, and case consultations on a variety of healthcare topics in the long-term care industry.

 

Jessica Wood, CTRS, CDCP

Jessica (McBride) Wood, CTRS, CDCP is a Certified Therapeutic Recreation Specialist (2003) with over 20 years of experience in Physical Rehabilitation, Substance Use Disorders, Mental Health, Long-Term Care Communities, and Skilled Nursing Facilities.  Jessica graduated from Illinois State University with a Bachelor's Degree in Science, majoring in Therapeutic Recreation with a minor in Gerontology. She is the recipient of the Gerontology Scholarship (2003). She is a Certified Dementia Care Partner.  Jessica has been a motivated and enthusiastic consultant with the SwCG team since 2022. 

 

TRACI L. PARETI, CTRS

Certified Therapeutic Recreation Specialist / Certified Dementia Practitioner / Instructor

With 28 years of experience in therapeutic recreation, Traci L. Pareti is a highly respected Certified Therapeutic Recreation Specialist (CTRS) and Certified Dementia Practitioner (CDP). Specializing in education and training for long-term care facilities, Traci has worked with both private and not-for-profit sectors, offering invaluable expertise in dementia care and program development.   Throughout her career, Traci has held key roles, including Dementia Special Care Coordinator and instructor for Dementia Care, where she has passionately contributed to improving the lives of residents through education. She holds a Bachelor's Degree in Recreation Administration from Eastern Illinois University and has developed deep expertise in serving geriatric and psychiatric populations in long-term care settings.  Traci’s areas of expertise include advancements in dementia care (with a focus on Person-Centered Care), program evaluation, regulatory compliance (both state and federal), recreation administration, and staff development. She recently launched a comprehensive 42-Hour Activity Director Course, which includes 6 hours of specialized dementia training, and she plays an active role in the Ohio Activity Professionals Bootcamp Workshop held twice a year.  A dedicated member of the National Association for Activity Professionals, Traci is also involved with various state healthcare organizations such as the Illinois Healthcare Association and Ohio Health Care Association.  Outside of her professional life, Traci is a passionate Chicago Cubs fan and enjoys spending her free time on the lake.

 

Sally MacLean, CTRS

Sally Maclean, MA, CTRS has been with SwCG, Inc. for 8 years since 2017. She is an experienced Recreation Therapist, Dementia Trainer and elder advocate. She has been in the field leading activities for over 25 years. Sally earned her BS in Psychology from Elmhurst College and received her Master’s Degree in Therapeutic Recreation from Aurora University. Sally began her consulting career after receiving her CTRS. Taking a break to raise her family, she returned to consulting in 2017 in the Illinois long-term care community. Sally has been a wonderful asset and addition to the SocialWork Consultation Group, Inc. family.

 

Theresa Milliron, CTRS

Theresa Milliron has been with SocialWork Consultation Group, Inc for ten years as an activity/therapeutic recreation consultant.  She received a BA in Psychology and MS in Recreation Parks and Tourism Administration with an emphasis in Recreational Therapy at Western Illinois University. Theresa worked at Children’s Memorial Hospital in the Child and Adolescent Psychiatric Inpatient Unit for 10 years. From there she transitioned to a career in nursing homes and assisted living facilities. Her work experience as a Psychiatric Rehabilitation Service Director (PSRD) and Activity Director for 5 years and then as a Memory Care Coordinator for 4 1/2 years has made her a knowledgeable and experienced asset to our team.  Theresa is a Certified Therapeutic Recreational Specialist for 20 years along with being a QMHP and a Certified Dementia Trainer.  She is also an active member of the Chicagoland Activity Professional Association.

 

Sheri Lewis, CTRS

Sheri Lewis, CTRS, is an experienced recreation therapist who has worked with the long-term care community for over thirty years.  She has enjoyed certification as a CTRS since 2007. She earned her bachelor’s degree from the University of St Francis in Joliet, IL in Recreation Administration. Sheri has worked in a variety of long-term settings including employment as an Activity Director, Memory Care Coordinator, and Recreation Therapist in a hospital-based setting. Sheri has recently transitioned to consulting for SocialWork Consultation Group, Inc. and brings a variety of diverse experience for the geriatric population. When Sheri is not working, she enjoys spending time with family, reading, crafting, and in the summer, gardening.  We are honored to have Sheri on our team!
